Pharmacist‐delivered comprehensive medication management in a substance use disorder clinic, an 18‐month descriptive study

Introduction Pharmacists have expanded their roles within interprofessional teams in substance use disorder (SUD) care, but data characterizing pharmacist-delivered comprehensive medication management (CMM) outpatient SUD treatment are limited. Objectives The purpose of this study was to characterize the patient population, and therapy problems (MTPs), describe considerations for CMM development implementation an setting. Methods An observational, prospective, descriptive conducted center. A clinical pharmacist provided services referred clients documented elements after each visit. comprehensively assessed clients' complete regimens optimize through collaboration with other healthcare providers. post-CMM electronic survey sent care assess attitudes collect feedback regarding process. Descriptive statistics were employed services, MTPs, responses. Results From April 1, 2021, September 30, 2022, a total 187 encounters delivered 112 SUD. Nearly all patients (99%) had opioid as primary diagnosis vast majority (95%) receiving methadone treatment. Clients’ mean age 39-years-old average eight medications six medical conditions. Of 201 MTPs identified, 54 (27%) marked “resolved” at follow-up visits. most commonly identified MTP category need additional drug (31%). team showed 100% agreed or strongly that consult helpful members. Conclusion Pharmacist-delivered setting feasible well perceived by members bridge existing gaps access biomedical optimization, coordination.
